I hereby request membership in the Catholic Daughters of the Americas through Court St. Anthony of Padua, #2651, Casa Grande, Arizona, and I declare that:

1. I am a practicing Catholic woman and a member of St. Anthony of Padua Catholic Church (or another parish).

2. I will strive to abide by the Bylaws, Rules, and Regulations of the Order.

3. I am at least eighteen (18) years of age.

🩺 Medical Emergency Release

I hereby give permission for emergency medical treatment to be administered in the event of illness or injury while participating in any activity sponsored by Catholic Daughters of the Americas, Court #2651. I understand that reasonable effort will be made to contact my emergency contact prior to treatment, and that I assume responsibility for any medical expenses incurred.
